
The Process
The process is approached by assessing the existing
supports, identifying needs of services, reviewing and developing a comprehensive person-centered care plan, and how to sustainably maintain it. From sharing granular needs to major needs, the goal is to cover as much information as we can during our meet to provide the best service(s) possible to help individuals live empowered and prepared. We will go through a "Molding Phase" as we continue to learn more on what is liked and disliked by the person receiving the services. The molding phase involves readjusting the services and program to further meet the clients personalized needs.

PERFORM

Conduct a preliminary research based on the clients needs

Create a short and long term curated program to be implemented

Create a transition plan

Establish agreed upon short and long term goals

Throughout the first 3 months of the program, make necessary adjustments to ensure the lengevity of the program.

ESTABLISH
Meet after the third month to further establish a plan (if needed). Doing this by reassessing and tailoring the ongoing and future needs. Decide to meet every number of months to continue the efficiency of the plan.

Understand the areas the individual would like to make progress in through LE&P coaching.

Customize how the 1:1 coaching will be setup within the individuals understanding to better teach, guide and instruct.
03.
Decide the number of lessons to setup each month. LE&P will test and perform the areas selected by the individual to have proven materials gathered, and provided for them to learn from in each lesson.
